Goals:
To provide residents with an introduction to the Fundamentals of Endoscopic Surgery (FES), including hands-on practice with the key skills required to pass the FES exam. This lab will familiarize participants with the tools, techniques, and principles of endoscopic surgery, and prepare them for certification.
Objectives:
Introduce residents to the FES program and didactic website.
Develop proficiency in essential endoscopic skills, including endoscopic navigation, injection, snaring, and loop reduction.
Familiarize participants with the FES training system to improve self-study efficacy.
Encourage friendly competition to enhance endoscopic simulation time and skill development.
FES (Fundamentals of Endoscopic Surgery) Resident Guide
1️⃣ Understand the Requirement
Why: Required by ABS as part of the Flexible Endoscopy Curriculum.
When: Often completed during or after your endoscopy rotation.
Components:
Didactic: Online modules (cognitive knowledge)
Skills: Virtual reality simulator tasks
Exam: Written + simulator-based skills test

3️⃣ Complete the Didactic Modules
The FLS didactics have moved and are now available to everyone, free-of-charge, at www.sages.org/owls.

Our residency programs is a certified testing center, and the testing happens up on the 12th floor in the sim lab.
Exam parts:
Written: multiple-choice questions (~1.5 hours)
Simulator: Timed and scored performance of core endoscopic skills
6️⃣ Test Day Tips
Bring a photo ID and your FLS voucher code to the test.
For simulator: keep movements smooth, avoid excessive insufflation, and follow the task prompts exactly.
For written: focus on clinical decision-making and complication management.
7️⃣ After the Exam
Results are typically immediate for simulator; written scores may take a few days.
Submit your certificate to your program coordinator.

You are the on‑call general surgeon. A 58‑year‑old man presents with melena, dizziness, and orthostatic hypotension. He has a history of daily NSAID use for osteoarthritis. Initial vitals: BP 88/54, HR 118, RR 20, O₂ sat 96% RA. Labs: Hgb 7.8, BUN 38, Cr 1.0, INR 1.0.
Scenario #2:
You are the on‑call general surgeon. A 64‑year‑old man presents with 4 days of progressive abdominal distension, minimal narrow caliber stools, and crampy lower abdominal pain. He continues to have some flatus. PMH: hypertension, type 2 diabetes. Vitals: BP 132/78, HR 96, afebrile. Abdomen: distended, tympanitic, mild diffuse tenderness without peritonitis. Labs: WBC 9.8, Hgb 12.2, electrolytes normal. Abdominal X‑ray: markedly dilated colon with inspissated stool, no free air. CT abdomen/pelvis with IV contrast: transition point at the sigmoid colon with an apple‑core lesion, no perforation, no distant metastases.
